Letters of Permission from Junior teams require the approval signature of the Hockey Edmonton Registrar, Glenn Sommerville. Scan the completed, signed Junior form, then email to: Glenn Sommerville. Or fax form to Hockey Edmonton office: 780-440-6475. The signed copy will be forwarded to the return email address provided on the form within a day or two.
